Camp Forms
Please note that all necessary forms that are not turned in when you register online should be brought with you to check in. However, please do contact us in advance if you have special medical needs.
- Camper Health History– please note that Health History Form 2 is not required
- Allergy Action Plan – only if needed
- Asthma Action Plan – only if needed

All required forms must be complete before we admit your child into camp. We must collect new forms for your camper each year; however, if your camper attends multiple sessions in the same summer we can use the same forms from one session to another.
Camper Code of Conduct
To ensure the safety and well being of every one of our campers and staff, we require campers and their parent/guardian to sign our code of conduct. While no list could be exhaustive, it shows the behaviors that are not acceptable at Camp Shaw, and consequences for those behaviors. Please ensure that you and your camper read and understand it, and turn it in online when you register for camp (one session per summer). Thank you for your cooperation.
