Forms & Paperwork: Fill out all necessary forms before arriving at camp. Permission slips and Code of Conduct are filled out online as part of your registration. The Camper Health History, including immunizations, should be brought to checkin with your camper, as well as the Allergy and Asthma action plans (only if needed for your camper).
